HOW CARE WORKS
Know what happens before support begins.
A clear process helps everyone understand needs, risks, responsibilities, costs and the next review point.

Initial conversation
We ask who needs support, where they live, what has changed, which tasks may help, preferred times and whether another professional is involved.
Home assessment
We meet the person, listen to their goals and routines, identify risks, discuss communication and consent, and understand the home environment.
Care plan and matching
The plan states the agreed tasks, timing, outcomes, risks, emergency contacts, recording requirements and how staff will be introduced.
Start and review
Care begins on an agreed date. Early feedback and regular reviews check whether support is effective and whether the plan needs to change.
WHAT WE WILL EXPLAIN
Before you decide
- What Civic can and cannot provide
- Proposed visit times and staffing
- How changes and cancellations work
- How care is recorded and reviewed
- How to raise a concern or complaint
- How personal information is used
WHAT WE NEED FROM YOU
Accurate information
- Current needs, diagnoses and relevant risks
- Medication and professional instructions
- Mobility, equipment and communication needs
- Key contacts and lawful decision-makers
- Access arrangements and home risks
- Any change after assessment
FEES AND FUNDING
Clear information before care starts
Care may be funded privately, by a local authority, through NHS Continuing Healthcare or through another agreed route. Eligibility and contribution decisions are made by the relevant funding body. Civic will explain its charges and the agreed service in writing before privately arranged care begins.
NEXT STEP
Start with a conversation
Tell us what has changed and what support may help. You do not need to know the name of the service before you call.
